Free motel check-in form and waiver

Both need a free account, which includes five documents a month with no card. “Use this template” opens it in GSD with the signature fields already placed; the download gives you the blank document to use however you like.

When you would use it

Independent motels and short-stay properties use a combined check-in form and waiver so the guest record, the house rules and the liability acknowledgement are all captured in one signature at the front desk, instead of a clipboard that has to be scanned and filed later.

What the document says

The guest identified below acknowledges the property rules for the stay described and accepts responsibility for the room and its contents during the stay.

What you fill in

Who signs

Terms included

The guest agrees to observe quiet hours, occupancy limits, and no-smoking rules posted at the property, and accepts charges for damage beyond normal wear caused during the stay. Vehicles are parked at the owner’s risk. Check-out is by the posted time on the check-out date.

Common questions

Can guests sign this on a tablet at the desk?

Yes. Open the signing link on a tablet, hand it over, and the guest signs in the browser. The completed PDF and its audit trail file themselves.

Does the waiver cover the pool or the parking lot?

Edit the terms to name the amenities your property actually has before you use it. A waiver that describes facilities you do not offer is worth less than one that names the ones you do.

Sending it for signature

Open the template in GSD, fill in your details, and enter the other party's email. They get a link and sign in their browser on any device (your signer needs no account and nothing to install), and both of you get the completed PDF with an audit trail recording every view and signature, plus a SHA-256 hash so any later change is detectable. Electronic signatures collected this way are intended to be legally binding under the U.S. ESIGN Act and UETA. See security & legal validity for the detail, and pricing for what it costs (the first five documents each month are free).

This template is a starting point, not legal advice. Read the terms and edit them to match how you work, and check that electronic signing suits your document type and jurisdiction before you rely on it.
