Free plumbing service authorization form
Both need a free account, which includes five documents a month with no card. “Use this template” opens it in GSD with the signature fields already placed; the download gives you the blank document to use however you like.
When you would use it
Plumbers use a service authorization on repair and emergency calls so the customer has approved the diagnosis and the price before the wall opens. On emergency calls in particular, having the approval on record is what keeps an after-hours invoice from turning into a chargeback.
What the document says
The customer authorises the plumbing contractor to perform the work described below at the stated price. If concealed conditions require additional work, the contractor will obtain approval before proceeding.
What you fill in
- Scope of work e.g. Replace water heater, 50 gal
- Price e.g. $1,250.00
- Access notes e.g. Side gate; dog secured
Who signs
- Customer: signs first
- Provider: signs after the customer
Terms included
Work is performed to applicable plumbing code by qualified personnel. Water service may be interrupted while work is in progress. The customer provides access to shut-off valves and the work area. Payment is due on completion unless otherwise agreed. The customer signs first; the provider countersigns to confirm the authorization.
Common questions
How do I handle an after-hours emergency call?
Send the authorization from the truck before you start. The customer signs on their phone in under a minute, and the after-hours rate is on the document they signed.
What if the job turns out bigger once the wall is open?
Stop and send a revised authorization for the new scope and price. One signed document per approved scope is the cleanest record you can have.
Sending it for signature
Open the template in GSD, fill in your details, and enter the other party's email. They get a link and sign in their browser on any device (your signer needs no account and nothing to install), and both of you get the completed PDF with an audit trail recording every view and signature, plus a SHA-256 hash so any later change is detectable. Electronic signatures collected this way are intended to be legally binding under the U.S. ESIGN Act and UETA. See security & legal validity for the detail, and pricing for what it costs (the first five documents each month are free).
This template is a starting point, not legal advice. Read the terms and edit them to match how you work, and check that electronic signing suits your document type and jurisdiction before you rely on it.
